The Essential Plan is a health plan for New Yorkers with no premium for those who qualify. It covers all of the essential health benefits covered under the Qualified Health Plans offered on the Marketplace

Adults may be eligible if they: are U.S. citizens or meet immigration status requirements, an adult who is 19 – 64 years of age, New York resident, and meet income eligibility criteria as shown below.

Eligible consumers will pay either $20 monthly premium or nothing, depending on income. Some plans include additional benefits such as adult dental and vision coverage for an additional premium. All New Yorkers should get tested for COVID-19. Tests are fast, simple, and free. Look for a testing site near your home. You will not be asked about immigration status. COVID-19 testing and care services are not a public benefit under the public charge rule.

What is an FQHC?
 
FQHCs (Federally Qualified Health Centers) are in most cities and many rural areas. They will help you, even if you have no health insurance. You pay what you can afford, based on your income.
 
FQHCs are open to all, regardless of their ability to pay. FQHCs use a sliding fee scale with discounts based on patients’ family size and income.

Free Medicaid coverage is available for pregnant women through the New York State Department of Health.
 
If you’re pregnant and uninsured, you may qualify to sign up for an insurance plan through the NY State of Health Marketplace, even if the enrollment period is closed.
 
Furthermore, free coverage is available regardless of immigration status. Babies receive health care services for at least one year after birth. Note: Extended access to family planning services is available to women who have Medicaid while pregnant but lose coverage after pregnancy.

HOW IT WORKS
Callen-Lorde provides all healthcare services regardless of ability to pay.
For uninsured/underinsured patients, Callen-Lorde provides a sliding fee based on your income. Visit co-pays can range from $0-$100. Please note that labs and prescriptions are not included in this co-pay. Any labs will be an additional cost, from approximately $50-$150.

The East Harlem Health Outreach Partnership (EHHOP) is the Icahn School of Medicine at Mount Sinai’s student-run, physician-supervised free clinic. EHHOP provides primary care to uninsured adults in East Harlem. Services are confidential and are provided at no cost.
 
Eligibility
To be an EHHOP patient you must:
– Live in East Harlem in the ZIP Codes 10029, 10035, 10128 at or above 86th St.
– Be 21 years old or older.
– Not have insurance and not qualify for insurance/Medicaid.

Primary Care, Women’s Health, Mental Health, Care Coordination, Ophthalmology, Cardiology, Podiatry, and Social Work.

Saturdays, 9 am – 12 pm.
Every Week: Medical Clinic, Mental Health Clinic, Nursing Care, Access to Care Team (ACT), and Social Work.
Twice per month: Women’s Health Clinic, Ophthalmology Clinic.

Services
Residents can access the Rx Card online for free and bring it to their local pharmacy for discounts averaging 44% off of prescription drugs. You can also search for specific medications to get an estimate on savings on their website. Please note that you do NOT need to enter a name or address in order to access the card – it is possible to just select the mode (text, email, print) and then click “get your free card” without entering personal information.
 
How to Use It

Residents can access the Rx Card online for free and bring it to their local pharmacy for discounts averaging 47% off of prescription drugs, with savings on generic drugs up to 53% and branded drugs up to 15%. How to Use It

Qualifications
Anyone in New York. There are no requirements needed to use the New York Rx Card program and they will not collect any personal information. Unlike insurance programs, there are no income, age, pre-existing condition, deductible, or waiting period restrictions. People with insurance may use the card as well for prescriptions not covered by their insurance or if the discount offers a price lower than their co-pay. You can ask your pharmacist for a price comparison.
