Services
Residents can access the Rx Card online for free and bring it to their local pharmacy for discounts averaging 44% off of prescription drugs. You can also search for specific medications to get an estimate on savings on their website. Please note that you do NOT need to enter a name or address in order to access the card – it is possible to just select the mode (text, email, print) and then click “get your free card” without entering personal information.

NYC Care is an option for low-cost or free health services in New York City. The services are available citywide for  New Yorkers who do not qualify for or cannot afford health insurance based on federal guidelines.

 

  • Keep in mind that NYC Care is not health insurance. NYC Health + Hospitals’ health care access program guarantees services to uninsured New Yorkers.
  • NYC Care offers a membership card, a regular doctor, high-quality customer service, affordable prescriptions, and more, as if it were health insurance.
  • There are no membership fees, monthly fees, or premiums. You will pay based on what you can afford.

The Essential Plan

 

The Essential Plan is a new health insurance program for New Yorkers. Working people with lower incomes who do not qualify for Medicaid can receive health insurance through the Essential Plan.

 

  • Applicants must be U.S. citizens or meet immigration status requirements. Also, be an adult who is 19–64 years of age. They should meet certain income requirements based on the size of your family.
  • View income levels for the Essential Plan at nyc.gov/assets/ochia/downloads/pdf/essential-plan-income-chart.pdf.
  • You will either pay no premium or $20/month and you will not have to meet an annual deductible.
  • Get hospital care, prescription drugs, and doctor visits as a minimum.
  • Enrollment is open all year. Call 1-855-355-5777 or TTY: 1-800-662-1220 to sign up.

 
Likewise, you can get free dental and vision care or pay for these benefits, depending on your income. Find more information about the Essential Plan at info.nystateofhealth.ny.gov/essentialplan.

The East Harlem Health Outreach Partnership (EHHOP) is the Icahn School of Medicine at Mount Sinai’s student-run, physician-supervised free clinic. EHHOP provides primary care to uninsured adults in East Harlem. Services are confidential and are provided at no cost.

To be an EHHOP patient you must:

For uninsured/underinsured patients, Callen-Lorde provides a sliding fee based on your income. Visit co-pays can range from $0-$100. Please note that labs and prescriptions are not included in this co-pay. Any labs will be an additional cost, from approximately $50-$150.

People who are pregnant can get free Medicaid care through the New York State Department of Health.

 

  • If you’re pregnant and uninsured, you may qualify. How much you earn will determine if you qualify. Call the Growing Up Healthy Hotline at 800-522-5006 for information to see if you qualify.
  • Coverage is available regardless of immigration status.
  • To sign up, go to the NY State of Health Marketplace at nystateofhealth.ny.gov even if the enrollment period has closed.
  • Call 311 for assistance with your application.
  • Your baby will receive health care for at least one year after its birth.

Medicaid recipients can have access to medical transportation at no cost such as rides to and from the doctor’s office, the hospital, or another medical office for Medicaid-approved care. Approval from a Medicaid caseworker, a ride service, or some other agency is needed.

HOW TO APPLY
— When a patient has a condition that necessitates a mode of transportation other than mass transit, the practitioner/case worker must fill out the Form 2015
— If patient can use public transportation, they can inquire about the PTAR (Public Transportation Automated Reimbursement) program to their social worker
— To schedule a pick up, patient or provider must call Medical Answering Services (MAS) at (844) 666-6270 or visit the MAS’ website at medanswering.com. The type of transportation must be specified bases on patient’s health status
— After approval, the patient receives a confirmation call from the transportation agency one day before the appointment. It’s imperative to answer this call
 
CANCELLATIONS
— To avoid being labeled a no-show, Medicaid enrollees or their designated agents must call the transportation manager to cancel their scheduled service
— If the appointment for which transportation is being cancelled and is rescheduled for the same day or next day, the Medicaid enrollees or their designated agent should contact the transportation vendor directly to notify them of the cancellation

Beneficiarios de Medicaid pueden tener acceso a transporte médico sin costo, como viajes desde y hacia el consultorio médico, el hospital u otro consultorio médico para recibir atención aprobada por Medicaid. La aprobación del transporte a través de un trabajador social de Medicaid, un servicio de transporte, o alguna otra agencia es necesario.
 
Para transporte de emergencia, los pacientes deben llamar al 911.

The NYC Health Department has birth records for anyone born in the city.
 
If you are the parent of a newborn the Health Department will mail a birth certificate about a month after the child’s birth at no charge. Call 311 if you haven’t received the certificate within four weeks.
 
If the hospital where your child was born made a mistake on the birth certificate, you must submit your application and the newborn certificate you received to the hospital if the child is less than 1 year of age. Call the hospital’s birth registrar for more information.
 
CERTIFIED COPIES
Online: Request a copy by going online to VitalCheck, the authorized third-party vendor by the Department of Health. The processing time is 10 to 15 business days. Check the status of your order at a816-evital.nyc.gov/eVitalVRRTS
By mail: Complete, sign and notarize the following document: nyc.gov/assets/doh/downloads/pdf/vr/birth1.pdf. Include a self-addressed, stamped envelope.
Mail your application package to the address on the top, left-hand side of the application. The processing time is four to six weeks.

All New Yorkers should get tested for COVID-19. Tests are fast, simple, and free and you will not be asked about your immigration status. Remember that COVID-19 testing and care services are not a public benefit under the public charge rule.
 
How To Get tested
Testing sites: Look for testing sites near your home at my.castlighthealth.com/corona-virus-testing-sites or text “COVID TEST” to 855-48.
At-home testing: At-home testing for COVID-19 is now available to all immunocompromised New Yorkers and those ages 65 and older. To schedule an in-home appointment, call (929) 298-9400 between 9:00 AM and 7:00 PM (7 days a week). For free USPS test kits, make an order at covidtests.gov.
