Early Intervention via Child and Family Connections of Youth Services Network

The Early Intervention program in Illinois is for families with infants or toddlers, age birth to 36 months, who have concerns about how their child is developing. A caring and knowledgeable Service Coordinator will meet with the family to explain parents’ rights, and help to determine what evaluations and assessments will be provided by qualified professionals at no cost to the family. If services are recommended, staff will assist in developing an Individualized Family Service Plan (IFSP) that will link the family to providers and supports they choose to help their child learn and grow.

Children’s Home and Aid of Illinois

Provides family centered counseling services for children and parents, including those involved in the child welfare system and foster care/adoption situations. These counseling services help improve the social and psychological functioning of children and their families and maximize the family well-being and academic achievement of these children. Also provides pregnancy and early parent counseling and support. Other services include adoption and foster care services, early childhood care (MotherHouse Crisis Nursery for ages 0-6) and education, residential services, and youth services. Most services are free to Medicaid-eligible children. Provides services to children ages birth to three years old and pregnant women. Program options include: (1) Center-based pre-school, part day and full day; (2) Child care through local partners; (3) Home-based services. Easter Seals Family Support Services

Provides a variety of information and support services to help families of children with disabilities. Assistance from staff is unique to each family and is based on the family’s needs and priorities for their child. Services may include parent support groups including Northwest IL Autism Support Group, Ogle County Autism Support Group, Down syndrome Support Group, and Family Support Information Group, as well as parent trainings, consultation, and technical assistance. Easter Seals Family Support Services Respite Program

Serves up to 10 families a month for a total of 20 hours per family. Families chosen on a first come/first serve basis with a wait list used if necessary. Provides a respite worker based on your input and needs for your child. All respite workers have the Illinois State Board of Education Paraprofessional certificate. Eligibility criteria: birth-21; medical diagnosis of a developmental disability; denial from DHS-DORS program or Milestone (listed below) and cannot be used in conjunction with either program; live Winnebago, Boone or Ogle counties. Easter Seals – Teen Family Support Program

The Teen Family Support program serves English speaking first-time pregnant and parenting mothers who reside in Winnebago County. Teen mothers must be 19 years of age or younger and pregnant or parenting a baby under 2 weeks old at the time of referral. Services provided include; home visitation, doula support, prenatal group, parenting group, and infant mental health counseling services. Our services promote healthy and stable families through the development of strong parent/child relationships and increased parenting skills.

MELD

MELD provides programs for young mothers and fathers (through 21 years of age) and their children through our MELD Moms & MELD Dads parenting and support groups. The groups meet weekly at local churches and in local high schools. Volunteers provide childcare and healthy snacks to ensure that these families are able to take advantage of our services. Families come together to discuss parenting issues and to share ideas with each other. Life skills, job skills training, GED tutoring and assistance in completing education, career planning and guidance, pre-employment workshops, and employment opportunities and job placement services provided in addition to parenting information and support/resources services. Also provides Transitional Living Program and an emergency shelter known as Trinity House (see separate entries).

Mother House Crisis Nursery (service of Children’s Home and Aid)

Offers 24 hour, 365 day/night nursery services to children, ages newborn to 6 years, for families in crisis. Families in extremely stressful situations can bring their children to Mother House for “free” temporary nursery care. Examples of crises may include violence, homelessness, addiction, hospitalization, NA/AA meetings, counseling, and stress. Provides situational crisis counseling, parent education classes, support groups and parent/child interaction groups. Conducts follow-up calls and home visits to help stabilize family, as well as referral and linkage to commuinty resources.

Teen REACH
Teen REACH provides FREE academic tutoring, cultural exposure, adult mentoring, fun field trips and recreation for youth ages 11 to 14 (6th to 8th grade). Teen REACH is an after school program held at West Middle School and St. Elizabeth Community Center.

Youth Achievers
An Afro-centric socialization program for Middle School youth ages 12-14. This program features interesting, challenging workshops and exciting fun filled educational field trips.

Youth in Action
Youth in Action is a program for boys and girls entering grades 6-9. the program promotes community service, leadership, and friendship. Program meets Mondays, Wednesdays and Friday 9-11:30am to complete community service projects. On Fridays, students attend fun-filled trips to various locations. The program is FREE of charge. Meeting location is I.D. Pennock Branch.

Learn to Earn
Learn To Earn is FREE program that provides extensive mentoring, counseling, job readiness, life skills, job shadowing and internships for in-school youth ages 16-18. This program also provides academic tutoring and helps improve study and social skills.

Link to Success
Link to Success is a FREE GED program for young adults ages 17-21 that includes academic tutoring, basic job and life skills training, and job shadowing.

YSN addresses mental health and trauma issues by providing counseling, intensive case management, mentoring, tutoring, transportation, and recreation in order to reduce the impact of abuse and life stressors on children and families.
