Replacement Glasses

Step 1:
Determine where child obtained glasses and whether eye exam was recent, either from the family or through MC+ plan.
Step 2a:
Contact Optometry office where exam took place and glasses were prescribed and request that they contact the specific MC+ plan and request ‘authorization for replacement glasses.’
Step 2b:
If the last eye exam was a year or more old, the child will need a new exam.
If parent does not remember what plan their child is in or what optometry office they went to, use the following information:

Will inform you which plan covers child. Have name of child, DOB, Social Security # (if possible). Plan can tell you where child was last seen and when last exam took place.
